Complex conditions care is a type of medical care designed to treat and support patients with two or more chronic illnesses that require ongoing support to maintain optimization but are not life-threatening.
Acute Care OnCall provides complex condition care services for illnesses likely to impact the quality of life and likely to last twelve months or longer. This includes regular and increased frequency contacts, visits, and check-ins. Your provider may discuss this option with you if multiple medical issues impact your life.
Specialized support care is mental, physical, medical, and emotional support and care designed to help individuals on their road to recovery from substances or behaviors.
Acute Care OnCall works with the client to examine all areas of need to avoid relapse when possible, address medical concerns while paying attention to recovery needs, and support existing mental health care for concerns like depression, anxiety, and others.
This includes regular and increased frequency contacts, visits, and check-ins. Your provider may discuss this option with multiple medical issues that impact your life.
Telehealth is the delivery of healthcare services and health care related information using equipment that can allow for communication between patients and healthcare providers and even healthcare providers exchanging information to provide better access to healthcare services. This work uses technology like the internet, satellite, iPhones, or smartphones. Telehealth services can take place in many situations, including hospitals, clinics, homes, and nursing facilities, just to name a few.
It is not always easy or convenient for patients to get help for their healthcare needs. The purpose of telehealth is to improve the opportunity to bring healthcare closer to home, in a convenient location, and in a more timely way. Telehealth is another option for receiving care from a provider.
The benefit of telehealth is less waiting and travel time and time away from work or other priorities. Examples of telehealth include a video “chat” for mental health care, evaluation of a sick child by video, or taking a picture of a skin problem and sending it to a skin specialist for assessment.

Acute Care OnCall provides urgent care services for adult patients aged 16 and older.
Most folks already have all the equipment they would need to enjoy the services and benefits of Acute Care OnCall; a wifi-enabled cell phone or laptop with camera and microphone.

Yes.
An in-person visit may be required from time to time, based on many factors like insurance requirements and your condition—no need to worry, though, in many cases, these in-person assessments can be scheduled well in advance and conveniently for you - even in your home or office!

All providers have completed training to the highest levels of their respective licensure, including national board certification.
Additionally, all providers have completed both the American Health Association's Telehealth Education Training and Stanford Medicine's Continuing Medical Education training.
Lastly, Acute Care OnCall has incorporated the US Department of Health and Human Services, Best Practice Guides for Telehealth, found at telehealth.hhs.gov in our practice to ensure the highest quality of care for our patients and clients.
